I understand and agree that Turpin Methodst Church is not responsible for accident or injury before, during, or after this event.

I understand that all booth hosts are responsible for bringing enough candy for at least 300 kids in the flavor of the assigned booth.

I would like to have someone assigned to my booth to host with me. I currently do not have a partner.

I understand that it's recommended that I dress to match my booth if I am a host.
